IVF with donor oocytes is one of the methods of in-vitro-fertilization, which has helped many couples to have a long-awaited and healthy child. This procedure is indicated in cases of female infertility caused by hormonal imbalance, genetic diseases, developmental disorders of the genital glands, etc.

Specialists at the reproductive health clinic “ECO” successfully fertilize donor eggs. Doctors take medical history, prescribe a comprehensive instrumental and laboratory diagnostics of the body, carefully select donors for oocyte donation. Such an approach at work increases the chances of successful implantation of the embryo and its further bearing.

IVF WITH DONOR EGGS

In vitro fertilization with an egg of other person refers to the artificial method of conception. The procedure is performed in case of diagnosed female infertility. During IVF the fertility specialist obtains the egg from the donor, artificially fertilizes it with the sperm of patient’s husband or her sexual partner, and then puts the embryo into the uterus of the recipient. The technique helps to have a healthy child, even in cases where other treatment methods have failed.

During the manipulation, the specialist selects an oocyte - an immature egg, taken from a donor's ovary. Fertilization occurs in vitro, outside the female body. After 3-5 days, the fertility specialist implants the embryo into the uterine cavity of the recipient. The procedure is performed on an outpatient basis in accordance with the norms of Order No. 107N of the Ministry of Health of the Russian Federation.

Important! For IVF with donor eggs both fresh and frozen biomaterial can be used. The technology of the procedure will depend on the chosen option.

INDICATIONS FOR THE PROCEDURE

IVF with donor oocytes is prescribed to women who have not had a conception after a year of active sexual life without contraceptives. The procedure has a peculiarity - the woman will not be able to transfer her genes to the unborn child. Therefore, it is prescribed in cases where other methods of conception have not given results.

IVF with donor egg is carried out in case of:

  • lack of oocytes because of early menopause, premature ovarian failure, menstrual disorder, adnexectomy;

  • pathologies of the genital glands caused, for example, by Turner syndrome;

  • diagnosed gonadal dysgenesis;

  • Genetic diseases related to the sex (Duchenne muscular dystrophy, fish-skin disease, etc.);

  • an unsuccessful attempt of standard IVF procedure, in which non-viable oocytes were obtained (from 5 cases or more);

  • a small amount of eggs, despite hormonal stimulation;

  • a woman is undergoing chemotherapy treatment.

IVF with donor oocytes is prescribed only after an accurate diagnosis. If the fertility specialist sees alternative treatment methods for infertility, the doctor will fall back on them.

REQUIREMENTS FOR SELECTED DONORS

Ovum carriers are women who voluntarily donate immature oocytes, which will subsequently be fertilized and implanted in the uterine cavity of the recipient. Serious demands are placed on donors. Applicants must:

  • be under the age of 35;

  • have one healthy child;

  • lead a healthy lifestyle (do not drink alcohol, do not take narcotic and psychotropic drugs, etc.);

  • have the conclusion of a geneticist about the absence of hereditary diseases;

  • work in harmless conditions;

  • have a psychiatric opinion confirming the absence of neuropsychiatric pathologies;

  • get a medical report from a gynecologist about the absence of diseases of the reproductive system.

Important! The donor must comply with all the requirements regulated by the Ministry of Health of Russia. If at least one requirement is not met, the woman is excluded from the candidates for egg donation.

THE LIST OF EXAMINATIONS FOR DONOR

The IVF procedure starts with preparation. At this stage, the donor has to undergo a comprehensive examination by laboratory and instrumental methods. A woman planning to donate an egg must undergo the following analysis:

  • CBC - a general blood test that reveals the qualitative and quantitative characteristics of the biomaterial;

  • UA- urinalysis is aimed at studying the organoleptic attribute of the fluid and sediment microscopy;

  • hormonal panel analysis - it reveals abnormalities in the indicators of estradiol, testosterone, AMH, LH, prolactin, thyroid hormones, etc.

  • coagulogram - this study allows to determine the speed of blood coagulation;

  • tests for the detection of syphilis, HIV/AIDS, hepatitis B and C, herpes and other infectious diseases of the genitourinary system;

  • Pap smear - it helps to eliminate dysplasia, detect morphologically altered cells.

This list of tests is mandatory. It may vary due to medical reasons, depending on the donor’s health condition. In addition, a woman who decides to donate an oocyte will be required to undergo instrumental diagnostics. It include:

  • Ultrasound of the pelvic organs - it reveals the pathology of the uterus, ovaries, follicles, fallopian tubes, etc .;

  • ECG - it is performed both in a relaxed state and under stress, helps to detect diseases of the cardiovascular system;

  • fluorography (lung x-ray) - this study is necessary to assess chest organs, detect tuberculosis, emphysema and other pathologies.

Examinations should be carried out by a physician, gynecologist, psychiatrist, narcologist.If there are any chronic diseases, the donor can be referred for examination to specialized doctors (otolaryngologist, ophthalmologist, surgeon, etc.).

HOW THE PROCEDURE IS CARRIED OUT

The steps involved in IVF with donor egg depend on how the manipulation is performed. In vitro fertilization can be carried out using both fresh and frozen material. In the first case, the protocol consists of:

  1. Comprehensive diagnosis of donor and recipient (they need to be prepared for the procedure).

  2. Synchronization of the menstrual cycle of two women (hormonal therapy is used for equalization).

  3. Oocytes retrieval.

  4. Egg fertilization outside the woman’s body.

  5. Cultivation of embryo and its transfer into the cavity of the reciprocal organ (uterus) of the recipient.

This IVF method has both advantages and disadvantages. The main advantage of the procedure is that the fertility specialist initially receives a viable egg. The disadvantage of the method is related to price. The cost of fertilization, carried out this way, is high.

Doctors also perform IVF with frozen donor eggs. In this case, the survival rate of the material after thawing reaches 98%. The IVF protocol with frozen eggs provides:

  1. Selection of candidates with a comprehensive examination .

  2. Hormonal stimulation of suitable donor.

  3. Retrieval and further freezing of material.

  4. Selecting a specific donor (the recipient makes it using a special cryobank catalog).

  5. Eggs thawing.

  6. Oocyte fertilization with the husband’s or partner’s sperm.

  7. Preparation of the recipient for implantation.

  8. Embryo transfer to the uterus.

This procedure is relatively inexpensive, especially in comparison with IVF with fresh donor oocytes. Fertilization is faster, because the recipient does not need to expect the results of the donor’s examination. However, the disadvantage of such IVF lies in the survival of the eggs, which reaches 98% after thawing.

SUCCESS RATE

According to statistics, IVF with a donor oocyte cannot give a 100% guarantee of pregnancy. The success of the procedure depends on many factors. These include:

  • recipient age;

  • general health condition;

  • the presence of chronic diseases;

  • improper lifestyle (poor nutrition, disturbed sleep patterns, etc.);

  • harmful working conditions;

  • severe diseases of the reproductive organs (endometriosis, adhesions).

The probability of pregnancy is higher among women aged 25–35 years who do not suffer from endocrine disorders, hormonal imbalances and pathologies of the genitourinary system. The success of the first IVF, according to statistics, reaches 30–40%. It is important to remember that, despite a comprehensive diagnosis and modern technology of the procedure, there is an unforeseen risk of embryo rejection. Pregnancy failure can occur due to the presence of antisperm antibodies in the recipient's body or a high concentration of embryonic killers.

The first failed attempt is not reason to stop treatment. Meeting all the requirements of the fertility specialist, changing the lifestyle and nutrition, women still have a chance to become a mother.

IVF WITH DONOR OOCYTES FOR PATIENTS OVER 40 YEARS

Couples over 40 years old are required to undergo a more intensive examination. A woman may be prescribed hysteroscopy to assess the state of the functional layer of the uterus (endometrium). A spermogram is indicated to a man, as it allows to determine the viability and motility of sperm. The recipient is also prescribed hormone therapy. This treatment is necessary in order to reduce testosterone levels and increase estrogen, progesterone, etc. Therapy is selected individually based on the results of a preliminary check.

PROS, CONS AND CONSEQUENCES OF THE PROCEDURE

IVF with donor eggs is a technique that has significant advantages. The main of them are:

  • the possibility of a multiple pregnancy;

  • sparing effect on the female body - the recipient is not stimulated;

  • the opportunity to become pregnant even with absolute infertility;

  • minimal consequences.

However, in vitro fertilization has also several disadvantages. It is customary to relate to following:

  • price - the procedure cannot be cheap, as it is notable for its complexity and high adaptability;

  • duration of the program if fresh oocyte is used for IVF;

  • a relatively low probability of success - on average it is about 35% (however, this is a good indicator for women with absolute infertility).

Despite the disadvantages, IVF with donor oocytes is widely used in reproductology. At the moment, this is practically the only way that allows women with absolute infertility to have a child.  In addition, this method of fertilization avoids the grave consequences after manipulation (hormonal imbalance, weight gain, early menopause, etc.). Side effects may include weakness and headaches. Only in 1 case out of 1000, according to statistics, uterine bleeding occurs.

ESTIMATED COST

How much is the price of IVF with donor eggs? Prices vary by regions and clinics. The cost is based on the number of prescribed examinations and the type of in vitro fertilization protocol.
